Step 1- New Customer Setup (3 Page Bundle)

A.)  Print and fill out all 3 sheets:  New Account Setup Bundle
 (Form E-595E, Tax Exempt Form, & Blanket Authority to Ship)

Please fill in ALL information from these forms.

You will need to Mail, Fax, or E-mail all of these forms to us.
This file is in PDF format and needs  to open them.

Step 2- Place Order

As you browse the site, write down the Style Number, Description & Quantity of the product that you would like to order. Click here to fill out our Order Form and when you are ready, click the submit button at the bottom of the page to send it to us. You may also Print and Mail, Fax or E-mail the order form to us.

Step 3- Confirmation of Order, and Delivery

We must speak with you prior to shipping to arrange payment terms and to check availability of items. We will contact you as soon as your order is received. Please double check your Account Form and Order Form, so that we make sure we have all of your information. We must have your New Account Form & Certificate of Exemption (form E595-E) before we can ship your merchandise.
